Neonatal:  5-10 mg/L

Critical value (automatic call-back):  >15 mg/L

Adults:  10-20 mg/L

Critical value (automatic call-back):  >25 mg/L

Physician Office Specimen Requirements

Peak
Container/Tube:
  Preferred: 4.5 mL Lithium Heparin (Mint Green top PST Gel) tube

Acceptable: 5.0 mL SST (Gold top) tube or 6.0 mL Serum (Red top) tube

Specimen:  Full tube
Collection Instructions:  For pediatrics, draw blood in a 0.5-mL green-top MICROTAINER®. Draw specimen 2 hours post dose, 4 hours post slow-release dose, or 30 minutes after intravenous dose.

Note:  1. Indicate peak on request form.

2. Label tube with patient’s full name and 1 additional unique patient identifier such as date of birth, medical record number, emergency ID number, or financial number. Date and time of draw, collector’s initials, and type of specimen are also required.

 

Trough

  3.5-mL red-top (gel) tube
Specimen:  Full tube
Collection Instructions:  For pediatrics, draw blood in a red-top MICROTAINER®. Draw specimen immediately prior to dose.
Note:  1. Indicate trough on request form.

2. Label tube with patient’s full name and 1 additional unique patient identifier such as date of birth, medical record number, emergency ID number, or financial number. Date and time of draw, collector’s initials, and type of specimen are also required.
