Physician Office Specimen Requirements

Container/Tube:  Lavender-top (EDTA) tube
Specimen:  Full tube
Collection Instructions:  For  0 to 4 years of age, draw blood in a lavender-top (EDTA) MICROTAINER®. If  >4 years of age, draw 3 mL of (EDTA) whole blood.
Note:  Label tube with patient’s full name and 1 additional unique patient identifier such as date of birth, medical record number, emergency ID number, date and time of draw, collector’s initials, and type of specimen.
