Physician Office Specimen Requirements
Submit only 1 of the following specimens:
Blood
Specimen must arrive within 24 hours of draw.
Container/Tube: 3.5-mL lavender-top (EDTA) tube(s)
Specimen: Full tube
Collection Instructions: For pediatrics, draw 1.5 mL peripheral blood in a lavender-top (EDTA) MICROTAINER®.
Note: Label tube(s) with patient’s full name and 1 additional unique patient identifier such as date of birth, medical record number, emergency ID number, date and time of draw, collector’s initials, and type of specimen.
Fingerstick Smears
Prepare fresh fingerstick smears (minimum volume: 1 well-made, unstained, peripheral blood smear from fingerstick). Forward promptly at ambient temperature only.
